Claude Code Prompt Patcher

Patches Claude Code's system prompts to fix two distinct classes of bad behavior:

  1. Corner-cutting / laziness — the original 11 patches. The model is instructed to be minimal in ~15–20 places vs only 3–4 instructions to be thorough (a 5:1 ratio favoring laziness). This rebalances that.
  2. Hallucinated facts presented as verified — 3 additional patches that force source citation when relaying factual claims from tools, subagents, and WebFetch results.

Corner-cutting fixes (1–11)

Source citation / anti-hallucination fixes (12–14)

These address a failure mode where Claude relays factual claims from tools (prices, API specs, quotes from web pages) as verified truth, when in reality the data came from a small summarization model (WebFetch) that can confabulate, or from a subagent that never actually verified the source.

When you relay facts from tool output, you MUST cite the source, distinguish verified from summarized claims, and never present WebFetch-summarized content as directly-verified fact without re-reading the raw page or corroborating from a second source. If a user challenges a fact, do not double down — re-verify from the source before responding."
14 Subagent summary Extends the Agent tool instructions with: "When the agent reports factual claims, your summary to the user MUST include the sources the agent cited (URLs, file paths, commands run). If the agent reported facts without citing verifiable sources, treat them as unverified — either re-verify before relaying or explicitly flag them as unsourced to the user. Never silently upgrade unsourced agent claims into authoritative statements."

What problem these solve

Real failure mode: a user asked for pricing data on Fal.ai models. An Explore subagent did a web search and returned "~$0.034/image". Claude relayed this as fact. When the user challenged it, Claude used WebFetch on fal.ai and reported seeing a detailed pricing table — which turned out not to exist on the page (the user could not see it in a browser). WebFetch's internal summarization model had confabulated the table. Claude eventually admitted "the WebFetch tool may have hallucinated those numbers."

The patches force the model to:

  • Always cite source URLs when stating facts from tool output
  • Treat WebFetch summaries as claims, not ground truth
  • Pass source attribution up through subagent summaries
  • Explicitly mark training-data facts as unverified (they go stale)

Version compatibility

Patches 1–3 are obsolete in Claude Code v2.1.100+. Anthropic removed the entire "Output efficiency" section of the system prompt in that release — so the target strings no longer exist in cli.js. The script detects this cleanly: those patches log SKIP (not found) and the overall exit still reports success.

Verified across bun binary versions:

  • v2.1.92 – v2.1.98: "Output efficiency" section present, patches 1–3 apply (2 matches each)
  • v2.1.100+: section removed entirely, patches 1–3 become no-ops

The remaining 8 corner-cutting patches (4–11) and all 3 source-citation patches (12–14) apply cleanly on v2.1.101.

If a future Claude Code release changes the wording of other targets, you'll see additional SKIP (not found) lines. The script warns when more than 4 patches skip (WARNING: many patches skipped — Claude Code may have changed its prompt format).

A/B Test Results (original patches 1–11)

Tested by giving both unpatched and patched Claude Code the same task: port box2d (30k lines of C, 56 files) to a pure JavaScript implementation with a working demo.

Unpatched (1,419 lines, 7 files)

  • Clean modular architecture (math, body, shapes, collision, world, index)
  • O(n^2) brute force broad phase
  • Sequential impulse solver with warm starting, Baumgarte stabilization, friction, restitution
  • Generic physics engine — only 2 references to box2d constants
  • No sub-stepping
  • Demo: click-to-spawn, pyramid button, rain button

Patched (1,885 lines, 2 files)

  • Single box2d.js bundle — 33% more code
  • Dynamic AABB tree (the real box2d broad phase)
  • Sequential impulse solver with warm starting, friction, restitution, soft contact constraints (b2MakeSoft formulation), sub-stepping (4 sub-steps for stability)
  • Uses actual box2d constants (B2_LINEAR_SLOP, B2_SPECULATIVE_DISTANCE, B2_MAX_POLYGON_VERTICES)
  • Demo: click-to-spawn, ramps, walls, pause/reset, performance stats

The unpatched version built a working physics engine but took shortcuts — brute force broad phase, no sub-stepping, no soft contacts. It's a "physics engine inspired by box2d."

The patched version built something closer to an actual port of box2d — dynamic AABB tree, box2d's specific constants and formulas, sub-stepping for stability, soft contact constraints. It did more work because the prompts didn't tell it to stop at "good enough."

The three highest-impact patches were #1 (simplest approach -> correct approach), #4 (don't add features -> fix related issues), and #7 (don't gold-plate -> work like a senior developer). Together they shifted the model from "build the minimum that satisfies the prompt" to "build something faithful to the source material."

How It Works

Claude Code ships as a Bun-compiled Mach-O binary with embedded JS bytecode. The binary can't be patched (bytecode integrity checks). Instead, this script:

  1. Installs the same version from npm (which ships as plain cli.js)
  2. Applies string replacements to the prompt text in cli.js
  3. Repoints the claude symlink to the patched npm cli.js

The --watch flag installs a file watcher (launchd on macOS, systemd on Linux) that monitors ~/.local/share/claude/versions/ for new binaries. When Claude Code auto-updates, the watcher automatically updates the npm package to match and re-applies patches.

Exit codes

  • --check: exits 0 if every patch that still has a valid target in the current version is applied (so skipped-obsolete patches do not cause a false failure), exits 1 otherwise. Safe to wire into CI or a health check.
  • Other modes: 0 on success, 1 on setup errors.

Safety

Before overwriting cli.js, the script copies it to cli.js.backup and verifies the patched version runs (node cli.js --version). If verification fails, the backup is restored automatically. --restore reverses everything: restores from .backup, repoints the claude symlink back to the bun binary at ~/.local/share/claude/versions/<latest>, and removes the watcher.
